What to include in a copyright notice

To help us review a claim under the Digital Millennium Copyright Act, 17 U.S.C. § 512, include: a physical or electronic signature of the copyright owner or authorized representative; identification of the copyrighted work; identification and location of the material claimed to infringe; your name, mailing address, telephone number, and email address; a statement that you have a good-faith belief the use is not authorized; and a statement, under penalty of perjury, that the information is accurate and that you are authorized to act for the copyright owner.
